What it does for you
Easy for your customers
Nobody should have to create an account just to leave a review. In the collection form, your patients or clients tap the stars first, then write a few sentences or record a video and add their name. You decide whether to ask for email, photo and job title as well. If writing feels hard, your own questions help on Pro, such as “What brought you to us?”. All built-in texts on the form appear in your space's language, English or German.
Consent you can prove
Before sending, every reviewer confirms your consent text; without the tick, no review goes out. Mocha stores the wording, version and time along with the IP address and browser user agent as proof. When you change the text under “Privacy”, a new version is created and earlier consents keep the wording they agreed to. Anyone who enters an email address gets a confirmation with a withdrawal link, and every reviewer can tick “Don't show my name”. You can download the consent log as CSV at any time.
Within reach everywhere
The form has a fixed address that you add to emails, appointment confirmations or your signature with “Copy link”. At the practice, a QR code at reception leads there, and on your website an iframe shows the compact card view right on the page. After sending, a thank-you page appears; on Pro you design it yourself, redirect to your website or offer a button to your Google review page. New reviews show up in your dashboard straight away.

Frequently asked questions
How do I put the form on my website?
Copy the iframe code on this page and replace “your-space” with your space's address, which is the last part of the form link you get with “Copy link” in the space navigation. The iframe always shows the compact card view. A plain link is even simpler: put the form link behind a button such as “Leave a review”, which works on every platform.
Does the form work on mobile?
Yes. The form adapts to small screens, the stars are easy to tap, and reviewers record video reviews with their phone's camera. So that this also works in the embedded form, the iframe code grants access to camera and microphone.
Can I change the look, the fields and the language?
Yes. Under “Design” you choose layout, style, colours, font and fields (email, title or company, photo); Pro adds your logo, the main colour, “Questions” as writing prompts and your own “Thank-you page”. You set the language under “Settings” > “Space language”, English or German, independently of your dashboard language.
How much does the collection form cost?
The form, consent with an audit log and the QR code as PNG are included in the free plan, with one space and up to 50 approved reviews, of which at most 3 videos. Pro costs €19 a month with a 14-day free trial and adds your logo and main colour, writing prompts, your own thank-you page and the option to hide “Powered by Mocha”.
How does the form handle consent and GDPR?
A review is only sent once the reviewer has confirmed your consent text; Mocha stores the wording, version, time, IP address and browser user agent as proof. Names appear in public only the way you set them under “Privacy”, and a withdrawal removes the review from every widget. The form pages run no analytics; whether your consent text suits your case is a question for your data protection adviser.
